Singapore Aims to Transform Community Hospitals into Population Health Powerhouses

Singapore is reimagining its healthcare infrastructure by expanding the role of community hospitals to tackle the growing pressures of an ageing population and the rising complexity of health and social needs. With the introduction of the national Healthier SG initiative, community hospitals are being positioned not only as intermediate care facilities but as central pillars in promoting population health across the country. The findings were published in the journal The Lancet Regional Health.

Traditionally, community hospitals have served patients recovering from acute episodes, offering rehabilitative and convalescent care. But with mounting strains on acute hospitals and the demographic shift towards a rapidly ageing society, authorities are now looking to community hospitals to deliver broader, more integrated services. These include preventive care, social prescribing, frailty management and even end-of-life services, all under a person-centred care model.

Under Healthier SG, Singapore’s three healthcare clusters are being tasked with managing population health as well as providing clinical services. Community hospitals, which currently manage about 4.5% of all public acute hospital admissions, are now being tapped for greater responsibilities. Among the proposed changes is the development of new models of care that emphasise continuity between hospital and community settings. This means redirecting suitable patients from emergency departments, outpatient clinics and nursing homes directly into community hospitals, bypassing overburdened acute facilities altogether.

Innovative approaches are being considered to make these transitions more efficient. These include early transfers of patients with defined conditions like stroke and hip fracture, expansion of allied health services, and the adoption of technologies such as virtual rehabilitation tools. Telemedicine and coordinated care plans are also being explored to ensure patients receive appropriate treatment at the right time and location.
